In order to receive a permit for waste collection or waste collection and transport, waste handling and storage, waste sorting and storage of waste storage, shall submit an application (in accordance with 2011 of 13 September of the Cabinet of Ministers No 703 4 of this Law).
In order to obtain a permit shall submit an application in accordance with the carriage of waste on 2011 September 13 the Cabinet of Ministers No 703 4) and (5), but if it is intended to transport hazardous waste, in addition to the information shall be appended to the submission referred to in Paragraph 6 of these Regulations.
Documents to be submitted: the application form for waste management licence in accordance with the Cabinet of Ministers on 2011 September 13 No 703 4, 5) and the requirements referred to in Paragraph 6.
The State fee for the issuance of an authorisation shall be paid in the following amount:
in the case of a permit for the collection and transport of waste in accordance with Sub-paragraph 3.1 of these Regulations - EUR 71,14;
for the authorisation of transhipment and storage of waste, EUR 71,14;
for the permit for the sorting and storage of waste, EUR 71,14;
for the authorisation of waste storage: EUR 71,14;
for an authorisation for the transport of waste, EUR 71,14;
The amount of the national fee for issuing an authorisation, which includes two activities, is EUR 106,72.
The amount of the national fee for issuing an authorisation which includes three (or more) activities (excluding the activity referred to in point 21.6) shall be EUR 142,29.
The national handover may be paid on-site and through a transfer.
